1 6.17.2005 Failure of the safety system circuit (Recommendation) At a U.S. nuclear power plant, the logic card for the safety system circuit failed to generate an incorrect signal, causing the reactor to automatically shut down. At this stage, the event is not considered to pose safety problems. However, electric power companies have been advised to be cautious about similar problems.  
2 8.18.2005 Modeling geometric regions in a criticality code (Recommendation)

Based on the NRC information on the possibility of a non-conservative error in modeling geometric regions in the Keno-V.a criticality code, an investigation was conducted on the current use of the applicable code by electric power companies, and found no problems. However, electric power companies and other associated businesses have been advised to remain cautious when using the applicable code in the future.
