On 28th Sep. 2005, the 76th Safety Caravan was held at Hamaoka Nuclear Power station, Chubu Electric Power Co., Inc. located in Omaezaki-shi Shizuoka Prefecture.

 

Safety Presentation

About 90 persons, including the employees of Chubu Electric Power Co., Ltd and cooperating companies, attended the Safety Presentation.


At the beginning of the presentation, Plant Manager, Junichi Ishihara, of the Hamaoka Nuclear Power station, Chubu Electric Power Co., Inc. greeted all participants saying, "At Hamaoka Electric Power station, safety is our first priority, but being satisfied, or thinking that we have done a good job, is taboo and improving safety even further is important. Today's Caravan is a good opportunity to stop and think about how we dealt with safety in the past and how we are going to deal with it hereafter which will in turn lead to safety improvement in the future.

Safety Presentation

After the address, the director of the Nuclear Safety Network Division (NS net) introduced the activity policies of the Japan Nuclear Technology Institute and gave an update of the NS net division's activities. After that, Director of the Quality Assurance Research Department at Japan Institute of Human Factors Co., Ltd., Mr. Kunihiro Watari gave a speech entitled, " Safety and the Human Factor"

During the lecture,

*When thinking about safety, it is necessary to know how humans process information. Humans have a single-information processing system and their processing ability largely differs according to consciousness levels. Errors can be minimized if one is tense to a certain extent but that state does not last for long.


*Humans do not set out planning to make an error, and a device that can reduce the occurrence of errors as much as possible by assuming and recognizing that errors are a natural part of human action is ecessary.


*When we look at the structure of consecutive accidents we see that first there is a history of success followed by overconfidence, conceit and pride which plant the seeds of destruction. Then when an accident occurs we think that it's a fluke and not a real problem. When the second accident occurs we think that it was due to a mistake by someone in the field and that everything is still okay and the problem is swept under the rug with passive countermeasures. As this pattern repeats itself and accidents keep occurring, we start to feel that what is happening is not our responsibility and employees lose their sense of motivation which eventually leads to a large disaster. Therefore, the following is important:

1;Throwing away your past honors, overconfidence and pride.
2;Changing failures (negative) into ways to prevent future occurrences (positive).
3;Not passing the buck.
4;Building an environment in which opinions can be openly expressed regardless of position.
5;Making safety a company priority.

 

Safety information exchange session

Safety information exchange session

The main information and opinions exchanged included:

(1)During activities introduction and the exchange of opinions at the safety information exchange session, Assistant Station Manager of the Tsuruga Power Station, Japan Atomic Power Company, Toho Maekawa, gave us a thorough introduction of the measures taken at Tsuruga Power Station to increase safety awareness, convey/maintain/improve technology, and build fair partnerships with cooperating companies. In addition, Chubu Electric Power Co., Inc. introduced methods for improving work transparency and building a feeling of unity with cooperating companies, education and training for maintaining/improving technological ability at Nuclear Training Center, and conveying experiences gained in the field using the "Learning by our Mistakes Corridor" which gives examples of past incidents. After that an exchange of information and opinions in regards to the above activities and measures took place.

(2)Other
The NS net division introduced a database and information concerning nuclear safety culture building that NS net has.
